This section provides general information about what types of services are covered with your specific plan. Click the red pins for details on each feature.
Gauges
1These show you how much you've already paid and how much you still need to pay to meet your out-of-pocket limits. Amounts paid for claims that have not been processed won't show up here.
Policy Drop Down
2You may have multiple policy numbers if, for example, you've switched employers or have a separate dental policy. The Policy drop down menu will list them all.
Member Drop Down
3If you have any dependents on your health plan, they will be listed here.
Date of Service Drop Down
4If your benefits change year-to-year, you can change the date of service to see what your benefits were at any point within the past two years.
Expenses Applied
5This section offers more detail on how much has been applied toward meeting your out-of-pocket expenses. If you have a family policy, you can see information for each individual, as well as a family total.
Privacy Authorization is Needed
6If a dependent on your health plan is over the age of 18, a privacy authorization is needed to enable you to view that person's information.
